Documentation Checklist Description
I. Loan Application:
o             Commercial Loan Intake Summary Profile Form – this includes a Use of Proceeds section – be as detailed as possible, and attach exhibits as necessary.
o             1003 – Commercial Loan Application PartB
o             Business Profile
o             Management Profile/Resume – please attach a
resume and list of projects completed or properties owned/bought/sold for each principal.  Please note the size and nature of the projects and include dollar amounts.
o             Business Debt Schedule
o             Please list all lenders who you have applied for financing with for this project.
o             General Signed Authorization and/or lender form
o             Current 12-months Business & Personal Bank Account Statements
II. Business Financial Information: Year 2017, 2016, 2015
o             Fiscal year-end Business Financial Statements (P&L and Balance Sheet), (Cash Basis) for this project only– for three (3) prior years. Business (1065, 1120, 0r 1120S)
o             Business Federal Tax Returns for three prior years, including all supporting schedules.
o             Interim Business Financial Statements (P&L and Balance Sheet), (year to date), (Cash Basis) for this project only-current within 45 days of application date.
o             Month-to-Month Financial Projections for the first 12 months following closing, and Annual Financial Projections for years 2-5.
o             Organization Documents – include a current Certificate of Good Standing, Articles of Incorporation, By-Laws, etc.
o             *Updated Personal Financial Statement
o             Organization chart with all employees and their compensation listed
o             Marketing materials used by the business
o             A business plan/executive summary or other narrative describing the operations of the Company, its future plans and how such plans are to be achieved
o             A list of competitors and industry trade associations
o             A narrative describing the operations of the portfolio company, the industry of the portfolio company, the pros and cons of the transaction and how the portfolio company satisfies  investment criteria;
o             Historical financial statements of the portfolio company (audited if available);
o             Information concerning the size of the industry and its growth potential
III. Personal/Principal Exhibits: Year Year 2017, 2016, 2015
Personal Financial Statement (form enclosed) for all partners, owners, etc.   These should be dated within the past 90 days.
o             Personal Financial Statement
o             Personal Federal Income Tax Returns (1040s) for three prior years including all supporting schedules.
o             Credit Report.  This can be ordered online at www.experian.com, www.transunion.com, or www.equifax.com.  Please include your credit score if you order these reports from any of these sites. Please explain any negative items on your credit.
o             Color (Copy) of Drivers license and or passport
IV. Property Information:
o             Purchase Contract, if applicable.
o             Settlement Statement for subject property
o             MAI Appraisal – if available.
o             Borrower’s estimate of the current As-Is value of the subject property, and basis for that determination.
o             Legal description and complete physical description of the property.
o             Environmental Reports – include any Phase I or Phase II Environmental Site Assessments, etc.
o             Color Photos of property, including aerial photos if available.
o             Maps of region, district and neighborhood.
o             Survey and Site Map (Plat) – if available.
o             Copies of pre-sale agreements/contracts if available
o             An architectural plan is a plan for architecture, and the documentation of written …architectural elements of a building project including sketches, drawings and details.
o             Construction/Renovation budget and time line for completion and copies of all construction contracts.  Please include resumes for the architecture firm and the General Contractor.
o             Permits/Zoning – provide all relevant permit/zoning documentation that relates to the subject property.
o             Documentation of equity invested to date in the project including original purchase price, cost of improvements, soft costs, etc.
o             Loan documents for all current mortgages encumbering the subject property.
o             Rent Roll (Form Enclosed)
o             If applicable, Schedule of REO Real Estate Owned and tenant occupancy history including rent rolls for the past 2 years. (Form Enclosed)
o             Current tenant lease summary: Start dates, expiration dates, security deposits, unit mix, etc. If this is for remodel or construction, need complete breakdown including money invested.
V. Affiliate Information: Year Year 2017, 2016, 2015
o             Income Tax Returns – previous three years with all supporting schedules
o             Fiscal year-end Business Financial Statements (P&L and Balance Sheet), (Cash Basis) – for three (3) prior years.
o             Interim Business Financial Statements (P&L and Balance Sheet), (year to date), (Cash Basis) – current within 45 days of application date.
o             List of ownership
o             Contact information for your Accountant and Attorney
